well guys initially i thought that this pic is fake simply because it really did not make any sense, i mean KAREENA, of all the people, doing what is an obviously deliberate expo.
but i happen to work at a place where its our JOB to detect the authenticity of some such pictures from time to time. I downloaded it and first ran a NEODEXTERO AUTHENTICATOR version 3, which gave a surprisingly high .98 value, (we accept values above .70 as authentic ) Then to crosscheck i ran the THERMAL PHOTO PIGMENT version 7 and it yielded a value of 89 on a maximum of 100.
so i guess its as authentic as it gets

Definite fake, and for the following reasons:
1. Kareena Kapoor's hands arent that shapely. (Weakness in argument: Could be an old pic)
2.The lower right(her right) transparent half of the dress has been morphed over the original image. The outline is blurred on the right (her right)
3.Could be that the whole image ha s been morphed. Her face looks extremely tiny in comparison to the rest of her, which is not how her body is proportioned, she has a robust face with a gargantuan jaw.
4.The boobs are small and too perky. Her boobs are big and pendulous, so much that they sag. (As seen on the latest Koffee with Karan)
5.The lighting and the blur on the right(her right) shoulder is not in sync with the lighting and exposure of the rest of the picture.
